Aside from the fact that just a few drivable vehicles completely change the dynamic of the game, there are other obstacles, such as map design.
Even if you were to stick vehicle driving in a few playlists, you'd have to completely rebalance the gameplay with strength of weapons, abilities, and far more open maps.
Just thinking about it quickly, on the available MW2 maps, where is there room to drive vehicles? All the maps are tightly packed with alleys, buildings and debris (though maybe Wasteland has room?). Driving vehicles in MW2 online would require a whole new set of maps with big open spaces, which in and of themselves would change the core tight FPS gameplay that is IW's signature.
FWIW, IW has never had drivable vehicles for online in any of their games, and I think this was the single biggest thing Treyarch got wrong in CoD:WaW. Map design has a huge effect on online gameplay, and IMO, this is where IW's work kicks Treyarch's ass all over the place.
I really like the maps in MW2 a lot. I think they have done a fantastic job with design and they all seem pretty balanced. I think that's one of the reasons I am enjoying this one so much online even with muted randoms.

Basically the L33T HaRDc0R3 g4m3rZ feel that using weapons like grenade launchers and rocket launchers require no skill and are only for newbies. I remember as far back as my days playing MOH:AA on the pc people calling rocket launchers newb-tubes. It's not as easy as it looks to use those weapons with a lot of success because using them usually slows you down a lot and if you miss you're essentially screwed.
